School of Nursing RN to BSN Preceptor Evaluation

Thank you for electing to precept one of Regent University’s students of Nursing in the RN to BSN practicum experience. Please read and respond to each question. If you have not directly observed the student in regard to the attribute listed in each section due to either lack of practicum experience to date or lack of exposure please mark the “Did Not Observe” box. If your student requires remediation in any section please mark the “Partially Met/Needs Remediation” box and provide a comment with specific reference to the observations of this student. Faculty will address all remediation needs with students. This survey will be open for one week to maximally capture your first exposure and experiences with your student. A secondary final evaluation will be sent at the end of the course in which you will document your final impressions of the student. If your student has not started practicum with you during this week you may evaluate based on your communication with the student i.e. have e-mails, phone calls and in-person interactions to obtain the necessary prerequisite paperwork for practicums been of a professional manner, has the student presented in professionally acceptable attire, based on your initial impressions would you consider hiring this individual, and any other insight that you would like faculty to have regarding this student.
