The Internship Contract must be completed along with the Individual Study Form for internship registration. The Internship Evaluation should be printed out in triplicate and given to your internship industry supervisor. This form should be completed by the industry supervisor at the end of your internship and sent to your faculty supervisor. The receipt of this form initiates a grade for the internship.
